The Medical Records Department will maintain a confidential, accurate and standardized chart for every person receiving medical services at the health center. These charts will be protected from loss, alteration, destruction and unauthorized and inadvertent disclosure.
General Facts about Medical Records:
Medical records are maintained in a secure electronic format and conform to all state and federal regulations for the storage of medical data. University Health Services maintains an accreditation by the AAAHC and follows all recommended medical records standards.
If you are age 18 or over, you are responsible for your own medical information. Only you can give us permission to release your medical information to another person; this includes your parents, family doctor, or professors. We cannot give out any verbal, written, or copied information without your consent, except in exceptional situations maintained by law. You have the right to view your record or have it amended.
University Health Services Medical Records Department establishes a medical file for each student upon arrival of their first scheduled appointment with a clinician. All student charts are kept for seven years after last visit. For a nominal fee a student may request their records by printing a release of information form, fill it out and send it to us by mail or fax. You may also come to the health center and sign the form.
A release must also be signed by the patient before our medical staff can speak to another person about you; i.e.: parent(s), professor, etc. The health center cannot release your records that have been sent to us from another facility. The patient must go to that facility to obtain those records.
