If your FAFSA was selected for verification, the One Stop for Student Services will notify you through email and/or your FlashLine account. We will let you know what forms must be submitted to our office. You need to submit a form only if it was requested by the One Stop for Student Services.  The forms are provided online for your convenience.

If your document needs to be returned in person, please check the library hours or with your Regional Campus student services area for hours of availability.  You can also return forms in a PDF format by mail.  Please see your form for details on if it needs to be returned in person.

These forms are related to the 2024-2025 FAFSA and for those students attending Summer 2024, Fall 2024 and/or Spring 2025.

2023-2024 Update:

Federal work study funding is limited each year.  New requests for Federal Work Study awards will not be accepted for any Kent State University campus for the rest of spring 2024. You may search for and apply for a university funded student job through the Student Employment Office in Career Exploration and Development.

2024-2025 Update:

Kent campus students: In early April 2024, the 2024-2025 Fall/Spring Federal Work Study Request Form will be available to request federal work study for fall 2024 and spring 2025. This form will be for both current students and new Kent campus freshmen starting in fall 2024.

Regional campus students: In early April 2024, regional campus students may begin to contact the financial aid office at their respective Kent State regional campus regarding a federal work study award for fall 2024 and spring 2025.

Federal Direct Loan CHANGE Request

This form is provided for students who are requesting additional Federal Direct Loan funds after the initial financial aid offer was processed.  Complete the Increase form if you need to increase your Federal Direct Loan amount or if you declined Federal Direct Loan funds and now want to accept them. Complete the Decrease form if you want to reduce or cancel your Federal Direct Loan.

Direct Loan Unveven Disbursement Request

This form is for students who need their federal direct Stafford loans to be disbursed in an uneven manner across terms.  For example, many graduate students have a stipend or assistantship that covers fall and spring tuition so, they may use this form to request a Summer Only direct loan.

Graduate permission for undergraduate courses

This form is for Kent State University graduate students who register for undergraduate courses that apply to their graduate degree. 

Federal regulations governing the Federal Direct Loan and Graduate PLUS Loan Programs require that:

• graduate students taking undergraduate courses must be enrolled at least half-time in courses that can be applied to the graduate program requirements.
• graduate students who register for undergraduate courses provide academic approval that the undergraduate course applies to the graduate program.

Federal Direct Parent PLUS*

In addition to the Federal Direct Subsidized and Unsubsidized Loans available to student borrowers, parents of dependent students are able to borrow additional funds under the Federal Direct Parent PLUS program. The PLUS loan requires that the parent complete and submit the PLUS Application. The parent information is forwarded to the Dept. of Education for credit evaluation.

Federal Direct PLUS for Graduate Students Application*

Federal Direct PLUS for Graduate Students is a loan from the Department of Education that allows graduate students to borrow additional funds to pay education expenses. The amount is determined by the cost of education less other financial aid including Federal Direct Loan eligibility. The Grad PLUS requires a credit evaluation. The Grad PLUS loan requires the student to complete and submit the PLUS Application.

If you have previously completed the Total and Permanent Disability Discharge (TPDD) process, the forms below are for you to regain your eligibility to borrow new student loans or TEACH. 

Please review the terms and conditions of your Discharge.  You may be subject to a 3-year-post-discharge monitoring period.  Some additional information can be found at Federal Student Aid.

Do I have to submit this form to receive any aid?

• No.  ONLY students wanting to borrow new federal student loans or TEACH should complete these forms.   
• Students who have completed the TPDD process and are not looking to borrow will be packaged based on their eligibility without federal student loans or TEACH.
  • Based on your federal eligibility, you may still be awarded Federal Pell Grant, Federal Supplemental Education Opportunity Grant, Federal Work Study and Merit Scholarships without changing your Disability status.

FEDERAL NURSING STUDENT LOAN

This form is for students enrolled in eligible nursing programs at Kent, Ashtabula, East Liverpool, Stark or Tuscarawas campuses.  Complete this form if you want to be considered for Nursing Loan funds in Fall and/or Spring.  Once you submit the form, it will be reviewed by the financial aid office for eligibility.  Please see the Nursing Loan eligibility criteria.

Private Loans

This form is provided for students who are requesting a change on the original request of the private loan requested with the lender in which they are borrowing.  The eligibility for a loan to be canceled or reduced depends on the timing of the request.  We cannot guarantee that a loan can be reduced or canceled upon request.

Cost of Attendance increase requests requires you to select the type of expense that you will experience. Expenses that will be considered are tuition & fees/room & board, childcare expenses, computer expenses (up to $2500 following purchaser’s receipt), and other educational expenses.  Please complete the Flight Cost of Attendance Increase Application if you have flight expenses.

Unusual Circumstances for Dependency Override

For students who: 1)  Have Legal Custodians other than their parents and have Court Documentation or, 2)  Students who have a unique situation (e.g., human trafficking, refugee or asylee status, parental abandonment, incarceration, both parents deceased) please contact the OneStop at www.kent.edu/onestop or your Regional Campus Representative to discuss your next step options.

Dependent Unsubsidized Verification

Students whose parents refuse to fill out the FAFSA on their behalf; or do not provide any financial support to the student, indicating the date the support ended, and will not provide any financial support to the student in the future may qualify for the Dependent Unsubsidized Loans only.

The student may complete the Dependent Unsubsidized Verification Form below to determine eligibility for the Unsubsidized Loan only (no qualifications for state or federal grants/scholarships).  If the parents refuse to sign and date the form to this effect, you must provide documentation from a professional third party (the student themselves is not sufficient) such as a teacher, counselor, cleric, court, or other professional.
