Who Is Eligible for "Know Your Numbers" Health Screenings?

The “Know Your Numbers” health screenings are for full-time, benefits-eligible employees only as part of the employee wellness and incentive program. Screenings will apply to their Tier 1 incentive for 2023. Make an appointment for your health screening and/or your flu vaccine by logging into the Be Well portal.  Once logged in, click on “Worksite Health Screenings” under “My Appointments”. First time user instructions can be found by selecting this link.
