 Apple Verification Form Payroll Deduction Form

All Full-time non-probationary Kent State University employees are eligible to participate in the Payroll Deduction program. This is an interest free loan available to purchase Apple Computers and Apple Accessories. All merchandise must be purchased from the Flash Technology Center.

PAYMENT SCALE

Initial 10% down payment required of total sale. $500 to $3,000 – 12 payments (6 months) deduction period or 24 payments (12 months)

Payment level will be approved upon salary verification from Payroll. Payroll deduction is only available for full-time Kent State University employees at the university for 6 months or more.

The Flash Technology Center will consult with the employee on their purchase and when they have made a decision, a formal quote and terms will be written. Payroll will be contacted for eligibility.  Once confirmed the employee will need to sign their Payroll Deduction Agreement. If the item is in stock, the purchase can be picked up at that time.

Once the purchase is completed, the Payroll Deduction Agreement will be forwarded to the payroll office to begin payroll deductions. Deductions will continue until the balance has been paid.
