If you were cited for Maximum Time Frame, you must also provide a completed Maximum Time Frame form.

Supporting documentation should be provided. The documentation submitted should support the extenuating circumstance stated in the appeal. Examples of documentation that support the circumstance are medical documentation, obituary or a counseling record. The dates of the occurrences on the supporting documentation should best coincide with the semester(s) the student struggled with academically.

This form is used for Kent State students to request a letter of financial aid for their visa applications as part of a KSU-affiliated study abroad program.

This form is to update your social security number (SSN) or individual taxpayer identification number (ITIN) in our system for the creating/processing of the Internal Revenue Service (IRS) 1098-T form.

This form is to request a stop payment on a check issued by Kent State University's Bursar office. This form is not for checks issued by BankMobile. Please contact BankMobile for stop payment information. 

This form is to notify the University that your Title IV Financial Aid can not be used to cover non-institutional charges for current and/or prior financial aid award years. Non- institutional charges include but are not limited to parking permits, service fees, and fines. Once the rescindment is in effect you will be responsible to pay these charges.
