KSU Ohio Sales Tax Exemption Certificate
This Ohio Blanket Sales Tax Exemption Form is provided for Kent State University employees who need to provide a sales tax exemption to a vendor for purchases by Kent State University. Please review Sales Tax Exemption Information prior to completing the exemption certificate.
If you are a vendor that requires a sales tax exemption certificate, please contact the Kent State University department that is making the purchase to obtain a signed form.
Instructions:
Please add the Vendor Name, Department address, sign and date the form. Anyone authorized to make purchases with Kent State funds may sign the form.
