"Know Your Numbers" – Sign Up Today for Health Screenings and Flu Clinics

As part of the Wellness Your Way incentive program, full-time, benefits-eligible employees are invited to attend an on-site health screening which will count toward their 2019 Tier One Wellness Your Way requirements.

 

In addition, flu vaccine clinics have been arranged to align with all upcoming health screening dates through December 2018 for all campuses.    

 
Completing Tier One Activities

Health screenings completed on-site with Be Well Solutions, Kent State University's wellness partner, automatically will be loaded into your personal portal. For health screening completed in a physician's office, employees must login to the Be Well portal, under "My Wellness," select "Wellness Reward Program," print the "Form 1-Physician Screening Form" and take it to the appointment. Completed forms should then be submitted to Be Well Solutions using the address on the form. Screenings conducted between Sept. 1, 2018, through April 30, 2019, count toward Tier One 2019, making employees eligible for their first $100 wellness incentive for 2019.

 

The following on-site dates remain to get a jump start on 2019 for “Know Your Numbers” health screenings and/or flu vaccines. Follow the “How to Schedule” instructions below as registration is required.

Only one health screening and flu vaccine clinic will be hosted in each Regional Campus. Employees are welcome to attend any clinic that best fits their schedule. But don’t delay, register today!

 

How to Schedule an Appointment

  • Once logged in, select "My Appointments," then "Worksite Screening." Choose the date and location that best meets your schedule. Please keep in mind that it will be necessary to fast for eight to 10 hours prior to the health screening, and blood draw is via a finger stick.
  • To complete/update the online health risk assessment for 2019 while logged into Be Well, select "My Wellness" and then select "Health Risk Assessment."
     
  • Results of each screening remains confidential at all times and will be provided directly from Be Well at the time of screening.
