This application is used by students who were dismissed at least twelve months prior and would like to return as an undergraduate student. Students can begin planning for reinstatement by reviewing the Reinstatement Policy in the University Catalog. If you answered “yes” to the question “Have you ever been convicted of a criminal offense or have charges pending against you at this time, other than a minor traffic violation?” on this form, you must also submit a Personal Statement for Undergraduate Reinstatement.
